Change of Ownership

A Change of Ownership Form should be completed and returned to the City of Sydney's Environmental Health Section when the following matters relate to your premises:

  • New Premises
  • Change of Proprietor
  • Change of Trading Name
  • Cease to Exist
  • Cease to Trade, or
  • Other.

The activities regulated by the City of Sydney Environmental Health section which relate to this form include:

  • Food Premises
  • Beauty Salon
  • Swimming and Spa Pools
  • Hairdresser
  • Skin Penetration (piercing, tattooing, acupuncture)
  • Places of Shared Accommodation.

Please note that a fee may need to be paid depending on the nature of your notification to Council.
